This information may include your name, email address, phone number, postal address, insurance information, personal health information (however this does not rise to the level of “Protected Health Information” protected by HIPAA), survey responses, user content stored or entered into the forms found in our Online Platforms, and other information you choose to provide through use of our Online Platforms.

Information Collected Automatically
